Advance Care Planning (ACP) is relevant at any stage of life, because unexpected illness or injury can happen to anyone. For older adults, ACP is especially important as health needs and priorities can change over time. It gives you the opportunity to think about what matters most to you, talk it through with family and whānau, and record your wishes for future health care.

Nelson Bays Primary Health offers Advance Care Planning info sessions to learn the basics, followed by Advance Care Planning workshops for completing your written plan. We are hosting these introductory info sessions at our Age Concern offices in Nelson and Richmond on a regular basis throughout the year ahead.

Having an Advance Care Plan in place can give you peace of mind, reduce stress for loved ones, and help ensure your preferences are understood and respected if you are ever unable to speak for yourself. We encourage everyone to have an Advance Care Plan in place and encourage you to attend a free information session to learn the basics, how to get started, and where to find support.

After attending an info session participants can join a dedicated workshop to work through a tailored guide, and capture what matters most to you should you become unwell and unable to speak for yourself.
